Tirzepatide/B12 is a once-a-week injectable dual glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) receptor agonist. It works by increasing insulin production and lowering glucagon secretion as well as targets areas in the brain that regulate appetite and food intake. This works in the body by: 

  • Delaying how quickly your stomach digests food, creating a sense of fullness and satisfaction with smaller meal sizes
  • Slows intestinal motility so you will feel fuller longer after meals
  • Dual action receptor allows for lower blood sugars, in part by reducing the production of sugar in the liver
  • Stimulates insulin secretion by the pancreas
  • GIP increases energy expenditure, resulting in weight loss reductions

Candidate Indications 

  • Tirzepatide/B12 is indicated as an adjunct to a reduced calorie diet and increased physical activity for chronic weight management in adults with an initial body mass index (BMI) 30 kg/m2 or greater (obesity).  
  • BMI 27 kg/m2 or greater (overweight) in the presence of at least one weight-related comorbid condition, such as hypertension, type 2 diabetes mellitus, or dyslipidemia.
  • Lab work must be presented to Medical Weight Controls staff within the first six months of start date, otherwise we must draw your labs in the clinic to continue injections. We will accept labs within 6 months of the visit. Labs include CBC, CMP, A1C, Thyroid Panel, Lipid Panel 

Contraindication 

  • Personal/family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. 
  • Type I Diabetic patient
  • Type 2 Diabetic patients must notify their primary care provider that they are starting Tirzepatide/B12 injection with MWC for weight loss management not DMT2 treatment.
  • Any patient under the age of 18 years old  
  • Any patient trying to conceive, known pregnancy, or lactating 
  • Known hypersensitivity to Tirzepatide, B12 or other ingredients. 

Common Side Effects 

Nausea, diarrhea, vomiting, constipation, abdominal (stomach) pain, headache, fatigue, dyspepsia (indigestion), dizziness, abdominal distension, eructation (belching), hypoglycemia (low blood sugar) in patients with type 2 diabetes, flatulence (gas buildup), gastroenteritis (an intestinal infection) and gastroesophageal reflux disease (a type of digestive disorder).
